CPT code 90832 reports individual psychotherapy lasting 16-37 minutes. It belongs to the psychiatry/psychotherapy category and carries no MDM requirement — code selection depends entirely on actual psychotherapy duration.

The 16-minute floor is the minimum billable threshold for standalone psychotherapy under CMS rules; a 15-minute session does not support any psychotherapy code.

90832 represents the shortest billable individual psychotherapy session, and its clinical niche is narrower than many practices assume.

Medicare and most commercial payers expect psychotherapy sessions to reflect genuine therapeutic engagement — not abbreviated check-ins that are actually medication-management encounters with brief counseling attached.

When a 2025 OIG psychotherapy audit flagged 111 of 120 claims as noncompliant, the failures were not duration-specific. They applied to 90832 as much as 90834 and 90837 — inadequate treatment plans, missing therapeutic-intervention documentation, and notes that failed to describe what the psychotherapy actually entailed.

The 16-minute minimum matters. A 15-minute interaction does not support 90832 — if no standalone psychotherapy code applies, the encounter may be better captured by the brief psychotherapy add-on codes (90833, 90836) appended to an E/M, or by an E/M code alone with counseling documented.

When should you use CPT 90832?

90832 applies when qualifying psychotherapy falls between 16 and 37 minutes.

Use CPT 90832 when

  • The provider delivers individual psychotherapy lasting 16-37 minutes
  • The service is standalone — no same-practitioner E/M on the same date
  • The service is medically necessary and connected to a behavioral/psychiatric diagnosis
  • The session constitutes genuine psychotherapy, not a medication check with brief supportive counseling

When not to use it

  • Psychotherapy lasted 38+ minutes — use 90834
  • Psychotherapy lasted fewer than 16 minutes — no standalone psychotherapy code applies
  • The same practitioner also performed E/M — use E/M + 90833 (the 16-37 min add-on)
  • The encounter was primarily medication management with incidental counseling — bill the E/M code alone or E/M + add-on
  • Group therapy occurred — use 90853

How is CPT 90832 selected?

One criterion — actual psychotherapy duration between 16 and 37 minutes.

Time thresholds

CodePsychotherapy time
9083216-37 minutes
9083438-52 minutes

The descriptor says “30 minutes,” but 16 is the billable threshold. Below 16 minutes, no standalone psychotherapy code is supported.

The same time-counting rules apply as 90834 and 90837 — count only direct therapeutic interaction, not charting, coordination, or medication management.

What documentation supports CPT 90832?

The documentation standard for 90832 is identical to 90834 and 90837 — shorter sessions do not justify thinner notes.

The note should include psychiatric diagnosis, actual psychotherapy duration, interventions used, target symptoms, patient response, treatment plan connection, medical necessity, and clinician signature.

The note must demonstrate that genuine psychotherapy occurred — not a medication check with brief supportive conversation.

Choose 90832 when actual psychotherapy lasts 16-37 minutes. Choose 90834 when actual psychotherapy reaches 38+ minutes.

The $42 gap creates a financial incentive to extend — or document extending — sessions past 37 minutes.

A practice where nearly every session crosses into 90834 territory despite scheduling 30-minute slots raises the same pattern-audit question as one that bills 90837 for most sessions.

What billing errors should you avoid with CPT 90832?

Four errors drive the majority of 90832 compliance issues.

1. Billing 90832 for a medication check with brief counseling

A 20-minute psychiatry visit focused on medication adjustments with 10 minutes of supportive discussion is an E/M encounter, not standalone psychotherapy. Bill the E/M code — and if 16+ minutes of genuine psychotherapy occurred separately, append the 90833 add-on.

2. Counting fewer than 16 minutes and billing 90832

Below 16 minutes, no standalone psychotherapy code is supported. Either extend the actual therapy or capture the brief psychotherapy via the add-on structure (E/M + 90833).

3. Inflating session time with non-therapeutic activities

Chart review, documentation, scheduling, and care coordination do not count toward psychotherapy time. The 16-minute floor must be met with actual therapeutic interaction.

4. Billing standalone 90832 with same-practitioner E/M

When the same provider performs both E/M and psychotherapy (16-37 min), the combination is E/M + 90833 — not E/M + standalone 90832.
