CDC/NCHS has released the FY 2027 ICD-10-CM files and CMS has released the FY 2027 ICD-10-PCS files, both effective October 1, 2026.
Separately, CMS Change Request 14464 revises diagnosis and procedure coding tied to several National Coverage Determinations. The NCD update carries an effective date of October 1 and an implementation date of October 5.
Nine days remain before the new code sets go live.
What the NCD revisions cover
CR 14464, originally issued June 2 and revised August 14, updates coding for the following NCDs.
- NCD 110.24 — CAR T-cell therapy
- NCD 110.23 — Stem cell transplant
- NCD 150.3 — Bone density studies
- NCD 220.6.1 — PET for perfusion of the heart
- Next-generation sequencing (NGS) and aprepitant are also included
- NCD 20.4 — Implantable cardioverter defibrillation (spell this out in internal references to avoid confusion with the ICD-10 abbreviation)
The August revision removed NCD 250.4 (Treatment of Actinic Keratosis) from the update. Practices that reviewed an earlier version of CR 14464 should confirm their coding references reflect the current list.

What remains the same?
CMS explicitly states that the NCD coding update does not change coverage policy.
The revisions are coding-maintenance changes — new, revised, or deleted ICD-10-CM and ICD-10-PCS codes mapped to existing NCDs.
A new diagnosis code on an NCD’s covered list does not create a new coverage indication; it reflects the updated code set within the existing framework.
Not every change affects every provider.
- ICD-10-PCS revisions concern inpatient procedure coding specifically
- NCD edits apply only to applicable Medicare claims within the affected coverage categories
- No new reimbursement rates or fee-schedule changes are included in CR 14464
Two dates, not one
CMS lists two separate dates for CR 14464, and billing teams should not collapse them.
- October 1 — the new ICD-10-CM and ICD-10-PCS code sets become valid for services rendered
- October 5 — the updated NCD editing instructions reach Medicare contractors

Billing teams should verify the following
The operational risk here is specific and preventable.
An outdated encoder or PM system running last year’s code tables on October 1 will reject valid FY 2027 diagnoses or miss retired codes — producing claims that fail before they reach the payer.
Practices that have not yet completed the following items are running short on time:
- Reviewed whether new and revised codes require changes to provider documentation
- Confirmed encoder and PM software are updated with FY 2027 ICD-10-CM/PCS
- Refreshed internal coding references and payer-specific guideline sheets
- Reviewed the addendum for new, revised, and deleted codes
- Test-run high-volume codes before the switch
The coding updates are annual, but the window for catching errors before they become denials is always shorter than it looks on a calendar.
